Step:-1 PERSONAL INFORMATION
Step:-2 SCREENING QUESTIONS
Step:-3 Education
Step:-4 DONOR CHARACTERISTICS
START OF PHASE II OF DONOR APPLICATION
Please answer all questions and do not leave any blanks. Be specific in your answers especially when completing the genetic section of the profile.Step:-5 RELATIONSHIP STATUS
Step:-6 EMPLOYMENT
Step:-7 GENERAL HEALTH INFORMATION
Step:-8 MENSTRUAL CYCLE
Step:-9 FAMILY GENETIC HISTORY
There is no perfect family history. In fact, most donors have cancers and other health issues in their family and still qualify for our program. It is critical to report all issues so a geneticist can compare your history with the male side of a prospective family to make sure that genetic diseases between your family and their's do not overlap which would increase the odds of passing a genetic problem to an offspring.Step:-10 FAMILY GENETIC HISTORY
Please list any birth defects or genetic conditions that have occurred in your immediate family – children, parents, siblings, grandparents and great grandparents, aunts, uncles, siblings and first cousins. All egg donors have an imperfect genetic health history. It will be important for future Intended Parents to understand any potential risks their children may have if they use you as an egg donor and compare that to the genetic risks on the Father's side.Fertility Heaven can not express how serious and important it is to talk to your parents to find out this information. It is rare for a donor to know the extent of family illness without talking to parents. A family is never disease or cancer free so we expect your profile to include various health issues.
Step:-11 PERSONALITY
Donors who take the time to provide thoughtful creative answers are likely to have more interest from recipients than those that write brief answers. Take your time and have fun writing about yourself. No one wants to learn more about you than the potential recipients who might have a child with your genetic material and think some of your special traits may be passed along in your egg.Step:-12 PHOTOS: WE CAN NOT ACCEPT YOU WITHOUT HIGH QUALITY PHOTOS. Donors with professional looking photos or excellent quality photos typically match very quickly and are in high demand. Should the photos not be acceptable for the program we will ask you to submit more photos. Photos are important to being matched.
Photos are very important in being matched. Upload flattering photos in good light. Do not upload photos with filters, hats, sunglasses or standing from far distances. Girls who are matched have great photos. I understand that the photos and the other information I provide through this application will be made available online for intended parents and third parties who are looking for egg donors. Please resize large photos. Ideal photos are 300 x 300 for uploading. If you have difficulty uploading photos email them to info@fertilityheaven.com.
